    • We’re reviewing some onboarding material for new techs and support staff who may need a quick refresher on ECG setup basics. One area that still seems to cause avoidable errors is chest lead placement, especially V1-V6. A few checkpoints we’ve been emphasizing are: V1: 4th intercostal space, right sternal border V2: 4th intercostal space, left sternal border V4: 5th intercostal space, midclavicular line V3: midpoint between V2 and V4 V5: same horizontal level as V4, anterior axillary line V6: same horizontal level as V4, midaxillary line One practical reminder that helps a lot is placing V4 before V3, since V3 is defined by the midpoint between V2 and V4. I also like having one reference that includes artifact troubleshooting and common placement mistakes, since poor tracings are often caused by setup issues rather than equipment failure.
